When Referred to KEIS

Soon after the child has been referred by the Paediatrician to Kurrajong Early Intervention Service the family is contacted by the Manager and an assessment date is set. The Family Support Worker then makes an appointment for initial contact to be conducted before the assessment takes place.

Initial Contact:

The family will be provided with information about KEIS and the service available to the family. At the initial contact meeting, the family shares relevant information about their child's background and needs.

Families will also be asked to sign a Conditions of Service form that explains both the KEIS service that will be provided and also the role that the family, will be expected to do.

One of the KEIS team members is appointed a Key Worker for the family. The Key Worker is responsible for facilitating and coordinating services for the child and family and is the main point of contact for the family.

The Next Step:

If the child is not yet 12 months old, the child will usually have individual sessions with the Physiotherapist, Occupational Therapist or Speech Pathologist as necessary. An initial assessment will take place once the child is 12 months of age.

Assessments:

If the child is over 12 months of age the family will have an assessment.

This assessment uses a transdisciplinary play based approach. At the assessment, the Physiotherapist looks at movement skills. The Occupational Therapist looks at fine motor and self help skills. The Speech Pathologist looks at developing communication and eating skills. The Educator looks at cognition (ability to learn), behaviours, socialisation and play.

In the last year of a child's involvement in KEIS, prior to their enrolment in school, a transition to school assessment is carried out which allows more specific information about the child to be recorded. This information can be used to help assist the school include the child successfully into the school setting.

Individual Family Service Plan (IFSP):

An IFSP will be developed with each family on an annual basis in January of each year. This IFSP is the plan of action for the child and family for the coming year. It will focus on what priorities your family, with the input from the team, would like to achieve. It details the level and type of service that the child receives for the year. This IFSP plan is reviewed at six months.
